Fall Immunization Reminder for future Kindergarteners and 6th graders!
Just a friendly reminder that the following grade levels require further vaccinations or a signed waiver for next fall:
4K going into Kindergarten: additional vaccinations of polio, MMR, and Varicella (chicken pox) are required so that a student has had 4 polio, 2 MMR, & 2 Varicella.
5th graders going into 6th grade: need a Tdap (adolescent tetanus, Diphtheria, and Acellular Pertussis).
Please plan ahead, have your child vaccinated this summer as there is always a rush right before school starts!
